Hi,

I am applying for PERM and I need to submit my experience letter from all previous employers which clearly states job roles and responsibilities along with other relevant docs. One of my ex employer is adamant that they cannot issue me any letter other than their standard format, which does not have job details.

I read in this case, I can submit an affidavit. Can someone share some details on this affidavit. I know the content but I need to know is it something like creating a stamp paper agreement by one of my manager in India and upload that? or is it something I need to create here in USA.

FYI: My manager too is also no longer employed with that organization.
