Regarding entry of petitioner name in h4 appointment scheduling

Hi there,

I am currently on H4 visa and looking forward to travel this May to India.

I need to get the visa stamping on the coming visit.

DS 160 has been filled.  moving ahead with scheduling the appointment for visa stamping using the below link:

i have confusion filling out the form.

I need to fill in the petitioner name and corresponding details.

Here do i have to fill in my details or my husband's details or my husband's employer's details.

Kindly assist me on this.

In my I797 , petitioner name is my name.
