Now while filling up the form I-765 they have a question Have you ever before applied for employment authorization from USCIS? No (Proceed to Question 12.) Yes (Complete the following questions.) Which USCIS Office? Dates Results (Granted or Denied - attach all documentation) ALso If I say yes (since I had OPT EAD) what dates should I enter...the start date or the start and expiry both?
