FOIA Request for I140 Approval copy

My employer is not sharing the I140 approval copy and so I want to request the document through FOIA - Online from USCIS website. Can you pls clarify the below

 

1) Is it mandatory to submit the G639 form (Through Online request)

2) If Yes, is it mandatory to do Notary in G639 form or optional

3) I do not have the I140 Receipt number also. So can I provide only my PERM A number?

No Notary needed here. Follow the Instructions and mention that you need everything related to the GC/I-140 petition of yourself. No guarantees. They are not supposed to give this though many have been lucky.

Can we request the I140 approval copy through USCIS website. I created the account and after login to https://first.uscis.gov I see the section to track the status and to link the existing case number. But I do not find any section to request for the I140 or other documents

 

Please let me know, if we can raise the FOIA request through USCIS website
