Move to US after a long unemployment in opt Stem extension period?

I am in India currently.
1. Graduated in STEM in May 2016 on F-1
2. Worked on OPT till June 6, 2017
3. Approved STEM Extension period: July 5, 2017 to July 4, 2019
4. For medical reasons, I returned to India in last week of July, 2017.

Is there a way that I can go back to States using my OPT STEM extension and look for a job there?

You have exceeded the allowable unemployment time and hence You cannot use this STEM OPT.

Most likely not. There is a limit of how long a person can be without a job on OPT STEM, I think 90 days, and that includes time abroad. In essence, your OPT is not valid anymore.
