However in her passport she got the last name added (we followed the name change procedure, posting ad etc in India), same name is on marriage certificate. When she filled her Ds-160 while coming to USA, she mentioned in the other name used as, lastName: Sapna FirstName : FNU. Her first name is Sapna but she had to mention this way as there was no surname used by her earlier.Now in the I-765 also there is a question Other names used, there is no separate fields for First and Last name.Should I mention her name as FNU Sapna or Sapna, in this section or should I not mention anything at all (just N/A.) This should be inline with what you have mentioned in the past ( in line to supporting documents too) If there are any corrections required, then first correct those.. This might delay processing of your application if the information is not correct or inaccurate.. Link to comment
