Proof of H4 approval document for applying H4 EAD

I was on H1 B status when i traveled to india. I abandoned my H1 and came back to USA on H4 visa (stamping). I don't have any document for H4 approval notice other than visa on my passport. 

 

below is the my visa status history

 

1) H4 Jun 2008 - Jun 2009

2) F1 July 2009 - july 2012

3) F1 OPT Aug 2012 - Oct - 2014

4) H1 Nov 2014 - June 2015

5) H4 July 2015 - current ( Got this stamped when i traveled to India based of spouse H1 b )

 

For H4 EAD application what document should i be sending as H4 approval document?.  Is the Change of status application still required?. Any other special considerations?.
