Filling DS160 with two valid H1B receipts

Hello,

 

1) My visa stamp is expired but my visa petition end date is only 30-sept-2013. I am going to India end of June 2013. So I need to appear for my visa interview and get stamping. My company processed my extension which is approved and starts from 1-Oct-2013. I want to get my visa stamped for the remaining time on my first H1 along with my extension. How do I fill my DS-160? 

2) I cannot enter both receipt's number in one application. Do I fill two applications?

3) In that case, how do I specify I have two applications while making my appointment?

Please help! I have my visa interview in 10days.
