Missing I94 with approved I797

My H1B was transferred my Visa from A to B

 

My last I797 and I94 expire date was 02/08/2016.

 

B filed my petition with premium processing and  and the shipment was delivered to Vermont Office on 2/4/2016.

 

My I797 got approved on Feb 22 2016. But I see that the I94 is missing with I797 

 

In the same I-129 petition , my spouse's I-94 was extended till 2/12/2018. when the same petition was filed for h1 and h4 ,

 

• I got my 1797 approved till DEC 2018 but not I94.
• My wife got I797 and I94 approved till DEC 2018

1. Is that a USCIS mistake? Is there a way that i can get I 94 without leaving the country?
2. What are my next options?
3. Am I out of status , since i do not have a valid I94?
4. What do you suggest i should do now?
