Wait for filing I-485 after completing 5 years in MUSA

I am a physician of Indian origin, and have already filed for NIW. I will complete 5 years in HPSA/MUA in July, 2014. Do I still have to wait for the priority date to become current after July, 2014 or can I immedicately file my I-485 after my 5 years are complete, even if my priority date is not current.Kindly advise regarding  the same.Thanks a lot.
