EAD/AP Combo Card Questions

Hi,

My EB3 Priority date is December 2003.

Renewing my EAD for 4th time now.

Our current EAD's are expiring in Feb 2013.

The new Combo Card which works as both EAD /AP.

1) Is there a special form or instruction page to apply for this Combo Card ?

Or is it just the regular I-765 and I-131 forms which need to be filled in ?

2) Do we have to send a covering letter to give a Combo card ? Or is it

upto USCIS to decide whether to give a combo card or EAD,AP seperately ?

3) How long is the validity of the Combo card ?

4) Did anyone get a combo card which is valid for 2 years ? or is it just 1 year ?

5) If the combo card validity is only 1 year, does applying for a non-combo card will

then get us an EAD which is valid for 2 years.
