What's the process to add last name during naturalization?

We are eligible for Citizenship and planning to file soon. We have a few questions 

1. My wife needs to add the last name, what is the process, and is it easy?  or do we need to go through Attorney?

2. Can we use a single online account to file an application for the whole of family?
