I-94 with approval notice is not getting matched with last port of entry.

I guys, 

I am holding L1-A and filing extension for my self and dependents. My wife has I-94 that got generated with last port of entry that is different from I-94 that she got with approval notice. 

However my son and mine I-94 are the same (port of entry-I94 and another got with approval notice)

Now I am applying for her extension, so not sure with I-94 record number I should use the one that got with approval notice or another one that is available on USCIS I94 official portal)    

 

Please suggest.
