GC Renewal questions on I-90

On the green card renewal application (I-90), there are following 2 questions,

1. Where did you apply for your immigrant visa or adjustment of status? Provide the location of the U.S. Embassy, U.S. Consulate, or USCIS office where you applied for your immigrant visa or submitted your application for permanent resident status.

2. Where was your immigrant visa or adjustment of status issued? Provide the location of the U.S. Embassy, U.S. Consulate, or USCIS office that issued your immigrant visa or permanent resident status.

Now, i had filed my GC through my employer in Arizona which was processed through the Dallas processing center at that time. So, is the answer to these questions = Dallas, TX or Arizona?
