Names mismatch

Hi All,

My parents are planning to visit US. The B2 Process is not started yet. The names in my father documents (Passport vs other supporting documents) are not matching (Both first and last name). 

 

Questions

1) In DS160, should they select the option as Yes for the 'Other names used' 

2) Should they go for the Affidavit (Notary), stating that, it's names mismatch but same person called with multiple names

2- No need. Passport is the main identification document, so they should use passport name as the name mentioned on DS160.

1) If they mention Yes for the question "Other names used'", will there be any issues for Visa stamping. For this any supporting documents required?

 

2) What exactly does this mean "Other Names used" in DS160 form.   Is it as per some official documents (like Passport, Nation ID card, Property/Business, etc...) names are different. Pls explain
