DS160 - H1b visa stamping

Hello!
I am currently in the process of completing DS-160 for H1b stamping. 
what address should I enter for my previous employer?

I was hired by a staffing firm (branch closer to my town ).
My W2s and paycheck indicate the address of the main branch.

Also, what should I enter for supervisor contact information?
My manager's information for the end client or contact information of the staffing firm.

Really appreciate your help.

Main branch address of your previous employer.

Staffing firm manager should be SUPERVISOR  info.
