WHICH I - 797 I HAVE TO USE

My old I-797 Valid until 30th sept 2018 and my new i-797 approved until 31 December 2019. I am planning to go for stamping on July 2018 as my status changed from F1 TO H1.  Which I-797 i have to provide in DS160( THERE IS OPTION TO ADD MULTIPLE RECEIPT NUMBER IN DS 160 )and US TRAVEL DOCS appointment slot details.

what is the Receipt Number(old  one or new one), start date and end date i have to provide in US Travels doc details ?

If i provide new I -797 which is future date will there be problem while entering USA because Visa is issued from Future date??
