Filling H4 EAD - Continuation Sheet

HI

I am filling I-765 for H4 EAD. In the instruction page it is mentioned as "If extra space is needed to complete any item, attach a continuation sheet ".

I am facing this issue in Item 1, for filling family name.

My question is 

1. In i-765 form what should i fill in the family name (space is not sufficient to fill the name). 

2. As extra space is needed i am attaching the continuation sheet for item 1. so shall i leave the family name with black space or do i need to specify any?
