Am I eligible for Interview Waiver Program for B2 visa?

Hello,

 

I have the following:

 

1. An unexpired H1B visa(stamp is expired though, however I have I-797 valid until 2017. I however quit my job and came back to India last year)

2. Visa is issued in India.

 

3.  My prior visa is not annotated “Clearance Received” or “Department Authorization”.

 

4. I have no visa refusals.(However, I applied a I-539 petition for change of H1B to B2 which was rejected. I wanted to travel in USA for 2 months after I quit my job. I applied the petition so that I may stay legally in the US as long as the status is pending. I left the country before the petition was rejected.)

 

5. My visa is granted when I was 21 years.

 

I now wish to apply for a B2 visa.

 

Please advise.
