How to confirm my current address with USCIS

I would appreciate some advice with my case here.

I am not sure if I file my last address change. How can I find what address dose USCIS has. My GC status will be current soon. So wanted to make sure this is correct.  

I am thinking of filing AR11 to be on safe side.  But  I noticed that the address change form has changed.

Now they are asking "Zip code used when filed". Not sure what should I put here.  Is this the state where I was working when my labor was filed ? will this create REF of it doesn't match with current zipcode?

Also not sure what to put for field "Specify if in the US as Other" that is under "Citizenship".  Will this be AOS?

Also am I correct that mail forwarding will not work of mails from uscis?
