L1 EAD ( Fresh or Renewal)

Hi,

 

I was on L1B and my spouse had EAD based on my L1B. The EAD for my spouse expired in 2013. We did not renewed after that as spouse changed status from L2 to H1b.

 

Now my spouse wants to change status back to L2 based on my new L1A status. My question is while filing i765, the category we should choose is a) Renewal of existing EAD b) Fresh EAD.

 

Also the Alien number should be same as old EAD?

 

What address shall we send i765 and i539 applications, we live in Wisconsin?
