smgc Posted November 11, 2015 Report Share Posted November 11, 2015 Hi All, Below are the list of documents that I am sending for myF1 --> H4 --> EAD concurrent application: 1) 2 Photos. 2) Two checks. One for $290 (I-539) and the other for $380 (I-765). 3) Completed form G-1145. 4) Completed form I-539. 5) Completed form I-765. 6) I-797A (received when changed status from H4 to F1) 7) Copy of Diploma received after completion of MS 8) All previously issued I-20s. 9) Latest EAD card. 10) Applicant's Paystubs for last 3 months. 11) Applicant's copy of passport data pages. 12) Applicant's latest I-94 card 13) Applicant's latest I-797A (H1B approval notice) 14) Spouse's approved I-797 (I-140 approval notice) 15) Spouse's paystubs for last 3 months. 16) Spouse's copy of passport data pages. 17) Spouse's latest I-94 card. 18) Marriage certificate Please let me know if what I have on the list here is good enough for F1 to H4 COS and H4 EAD. I am not including birth certificates as they don't have any photo. Please let me know if I should still include it. Also, do I need to send all I-20s or just do I just send the first I-20(one that I received when changed status from H4 to F1. This I-20 has D/S stamped on it) and my latest/current STEM OPT I-20? I have changed quite a few employers and have those many I-20s during the entire duration of my OPT. Link to comment
