legal name change prior to marriage & birth certificate incomplete information

Hi,

i am eligible to file for AOSwith the recent move to July 2011 for filing date of adjustment of status. As part of that, I was checking on documents for the application and had some questions.

My wife had her name changed before marriage. Her birth certificate ( as is the norm for most India folks ) with her prior name is also not complete. I am planning to have the followign copies for proof and wanted to know if I am missing anything

1) birthcertificate with her original name but has incomplete information ( no mother maiden name etc..)

2) Gazette print of the name change to new name

3) Affidavit of the Birth Certificate from her parents with the new name and also mention that she had changed it from her old name there. ( Is this correct)

4) Affidavit of name change with the citation of Gazette copy as proof.

 

Am I missing anything else? Also, is (3) above correct to get the affidavit on the new name and mention the older name also as other names that were used.
