Confused with dates while booking Visa appointment

I got H1 with start date as Oct 1st 2014 - XXXX 2017

 

I have applied for amendment in June 2015 and now, I got a new receipt number with new dates starting from June XX 2015 - XXXX 2018

 

While booking the Visa appointment,

 

1. I have to give amendment receipt number and dates mentioned in the amendment

 

2. Amendment receipt number, but start date as Oct 1st 2014 - XXXX 2018

 

3. Initial H1B receipt number and those dates.

 

I am assuming 1st one would be correct, please correct me if I am wrong.
