After that I went to India, got the J1 visa stamp on my passport and came back to US. I am trying to apply for J1 waiver and have the following questions about the form DS-3035: 1) Item 13 - Is there any period of time in the U.S. that is not covered by your form DS-2019/formerly IAP-66? Should I answer yes and explain that I was an F1 student? 2) Item 17 - Date and place of first entry into the U.S. on your original exchange visitor (J-1) visa. Entry information should refer to the first time the J-1 visa was used to enter the US. If EV changed to J-1 visa status while already in the US, enter the date of status change, control number and issuing post of the first J-1 visa. Since I changed from F1 to J1 while I was in US, should I leave the "Port of entry" box blank and fill in the rest of the information? Any help would be much appriciated. to the first time the
anandban Posted September 4, 2015 Author Report Posted September 4, 2015 Hello, I tried filling the DS-3035, but got an error message. The "port of entry" is a require field, I cannot leave it blank. Furthermore, there was no box to enter the "visa control number". Can anybody help?
