H1B to H4 - Please help filling I-539

Hi,

I need to file COS for my wife and in the process of filling I-539. I got stuck for few fields in the form and really need some advice.

My wife is currently on H1B visa which is valid till Aug 2016. She needs to leave her job for some reason and file COS to change status to H4, as a dependent on my H1B. 

Below are my doubts:

Part 1. Information About You

11.c Travel Document Number - where can I find this number?

Part 2. Application Type

2.a.  A change of status. The new status and effective date of change. (mm/dd/yyyy) 

 what does it mean by this date? she is still working and planning to quite in 2-3 weeks.

Part 3. Processing Information

 1.a I/We request that my/our current or requested status be extended until (mm/dd/yyyy)

   what does it mean by this date?

1b Check this box if you were granted, or are seeking, Duration of Status (D/S).

In which case do I need to check this box?
