H4 EAD - Filing Questions

Hi Guys, 

I know most of you have already filed and can answer my questions based on your experience. I am just starting the filing and need your inputs. Thanks for taking time to read and respond: 

1.  I-765 Questions: 

a. I worked on L2-EAD before and I am sure I need to include those details n this form. If not, let me know. 

b. Once I say Yes to question, "Have you ever applied EAD before", it asks "Which USCIS Office" & "Dates" - What should I fill in these columns ? 

c. For #10, Alien Number or I-94, which one to provide as I had A# in old L2-EAD card 

d. For #5, Birth State & City, I am born in Hyderabad, which is now under Telanaga. But I dont have any documentary proof for Telanaga, everything from passport or other documents has Hyderabad, AP. Can I still use Andhra Pradesh as state. 

e. For #13, Place of last entry: Can i fill in what it shows in I-94 online history, or shall I use mention City/State as online it is showing airport name as Place of last entry 

2. Does any documents needs Notary ? 

3. Can we staple all supporting documents ? and any documents with multiple pages (passport front & back pages) 

4. Do I need to include all Non-Blank pages of passport or just first & last page. Also my VISA in passport is expired and I am on I-797 extension 

5. Shall I place an Checklist document along with all supporting documents so that they know what is all included in the envelope ? 

6. Do I need to fill G-1145 form and send ?
