AR-11: Do I need to report all previous address changes?

I have moved twice since I came to the US and this is my third residence.

I have never reported the address change via the AR-11 form.

Now I am planning to inform about the address change to make sure I do not have any bad documentation in my record.

 

1) Do I need to declare both the address changes or just the last one?

 

2) Will I have to a pay a fine of $200 since I did not report the address change within 10 days of moving?

 

3) What other problems can you foresee?

 

P.S. I have my I-140 approved from a previous employer but my current employer has still not begun the PERM/LCA for me for GC.
