URGENT - H1B Amendment required??

Hi, here is my situation:

 

1. H1B petition was filed for me by my employer through Client A in advanced degree quota. It was approved but change of status was denied.

 

2. As my project ended at Client A, I moved to Client B and continued working on valid OPT for 6 months.

 

3. With new LCA for Client B and NO h1b amendment, I left USA to get the visa stamped.

 

4. I successfully got the visa stamped with Client B LCA at US Consulate in India and returned to USA to start working at Client B. 

 

Here are my questions:

 

1. Should my employer file h1b amendment based on Simeio Solutions Decision?

2. My extension is due in 2 months. Is it enough just to file for extension and not amendment?

3. If amendment is required, do we need to file amendment and extension separately? If combined, can you please let me know the category?

4. If separately, can we file for extension while amendment is in process?

5. How much is the application fee for Amendment and Extension?
