H1b Premium Process suspension- H1 transfer on approved I-140

Here is my case:

– I have completed 8 years on H1b (last two extensions were 1 year each and for company ‘A’).
– Now, I have my I-140 approved for company ‘A’ and
– Recently got 3 year H1b extension for company ‘A’ ( valid till 04/2018).
– My stamped VISA has expired
– Now, I have a full-time offer from company ‘B’

Having said that:

1. Can company ‘B’ apply for H1b transfer under premium processing (without extension)?

2. Can company ‘B’ apply for H1b transfer under premium processing (with 3 Year extension)?

3. Can company ‘B’ apply for H1b transfer without extension under Premium processing-Consular?

4. Can company ‘B’ apply for H1b transfer with extension under Premium processing-Consular?

Note: I have planned a travel in the first week of July-2015 as its school break for my daughter.
I have to take up Full-time with Company ‘B’ and do a premium processing so that I can go for vacation to India.

I would greatly appreciate your response and suggestions any you may have.
