H1 to H4 EAD filing questions

Hi all,

 

I am filing for H4 EAD and COS to H4 from H1 status.

 

1. Is the answer to question no. 16       © (26) ?
2. My I-94 number on my I-797 (H1B) is different than on my recent online I-94. Which one to enter in the I-765 form ?
3. What is A-number and where can it be found ?
