H1B amendment partial work from home and office

Hi

 

My LCA was initially filed for my workplace which is in FL(in 2012) but now I am work from home(in Indianapolis) for 2 months and I am in office for 2 weeks. Do I need to apply for amendment. 

 

My renewal is coming up in 2015. In LCA they are applying both work from home and my office as my Location. 

 

Please suggest.

The employer needs to file a new LCA and amend the H1.
