H4 EAD I-765 Questions

Hello Folks,

 

I have few questions.

 

My Wife is filling I-765 Form for EAD.

 

We have doubts for questions - 11 till 16 in I-765.

 

She entered US on B1 before marriage few years ago  and has no information on which USCIS office did her company send the form or apply to for the visa.

 

 

Following is the question in I-765 Form for your ref.

 

Please suggest the apt response for the following questions in this scenario.

 

Have you applied for employment authorization from USCIS ?

 

If yes answer the following questions else proceed to 12:-

 

11.Which USCIS Office?

 

    Date(s):

 

   Results; GRANTED OR DENIED. Attach Documentation.

 

12. Date of Last Entry in US:- 

 

13. Place of Last Entry in US:-

      

14. Status at Last Entry:-

      Should it be B1 OR H4

15. Current Immigration Status:-

       H4

16: Eligibility Category for filing I-765:(in the instructions the eligibility category is not mentioned. Please Advise.)
