I-765 Form for H4 EAD

Hello Folks,

 

I have a question regarding H4 EAD form:-

 

My Wife is filling I-765 Form for EAD.

 

In one of the questions it is asked if she has applied to USCIS for Employment Authorization. 

 

She has a H1B Stamping in 2008 from Satyam. However, she does not remember to which USCIS office she applied and the date.

 

Following is the question in I-765 Form.

 

What would she answer for the question 11. 

 

Have you applied for employment authorization from USCIS ?

 

If yes answer the following questions:-

 

Which USCIS Office?

 

Date(s):

 

Results; GRANTED OR DENIED. Attach Documentation.

H4 EAD form is not yet released by USCIS.

H1B is different from Employment authorization. You don't fill 765 for H1.
