H4 EAD application

I have 2 questions:

 

1. I read somewhere that the official EAD application for H4 is not yet published. Is this true? If so, when can we expect it?

 

2. In the existing, I-765 application, Question #16: 

Go to the “Who May File Form I-765?” section of the instructions. In the space below, place the letter and number of the eligibility category you selected from the instructions. (For example, (a)(8), ©(17)(iii), etc.).

 

What should we enter for the letter and number of the eligibility criteria?

Wait till USCIS releases the updated EAD application form.

Now that USCIS has released FAQs, and in the FAQs they have mentioned the I-765 form to be used.

 

1. Can we use the existing I-765 form or do we need to wait for a new one?

 

2. What is the category to be filled out? Is it ©(26) ?

 

3. What is address that we need to send the documents?
