H1B Extension beyond 6 years

My Perm was applied on Nov-2014. Its still under process.

My H1B is going to be expired on Oct-2015. In a worst case scenario, if my Perm is not approved by that time[either due to a query received or processing time delay], can I extend my H1 after Nov-2015, considering 365 days rule ? Please advise.

At the time of filing a H1B extension your PERM must be pending for 365 days.

Can a H1B be extended, which has exhausted 6 years, with Perm processing pending more than 365 days but I140 yet to be approved.
