h1b visa - ds 160 form query

Hi,

 

I have got my visa extended in US and now travelling to India to get it stamped. I have few queries on filling the ds-160 form

 

h1b self :

Salary in Local currency : Should i convert my USD salary to INR ? If yes, what should be the exchange rate. It doesn't take characters, so I cannot mention that it's in USD.

 

h4- my dependents:

Application receipt/petition number :

    I assume I need to enter the receipt# which I have received when I got my h1b extended in US (which is yet to be stamped in passport).

 

US point of contact : Can I enter my info / should I need to enter my reporting manager info ?

 

Please confirm the above.

Monthly Income (in USD): is how it shows on DS-160 so i don't see why you are trying to convert.

H4 petition number should be your exteneded H1b i797 number

us point of contact should be your employer.
