Transfer from a cap exempt to cap subject employer.

Hi I am a foreign medical graduate, and have been on h1B at academic places for 3 years through a cap exempt H1B. Now i am planning to get a job at a for profit organization. I assume that my H1B transfer will be under cap . My start date for the job is 1 August.

Following are my questions:

1) Can I start working on 1 August on the basis of portability law?

2) If not is there any way I can start working on 1 August?

3) If I apply for a premium processing, will that help?

4) If I get my visa approval, do I have to leave the country between August 1 to October 1?

5) How can I apply for a concurrent H1B?
