H1b amendment

I am working on H1b from 7 yrs with 140 extention, when my h1 extention was applied provided/mentioned Client-A address/ location, now I am working for client B, amendment/LCA was not applyed from employer for new location/ client B , now there is some medical emergency and need to travel to India, is this a issue for visa stamping in US counsalate in India , since i129 mentioned client A address.

For H1B visa stamping you need H1B amendment approved.

Obviously you need H1 amendment, tell your employer to file it under PP.

I heard amendments are gonna be straight forward but in worst case scenario, what happens if H1b amendment is denied? Would Initial H1b be still valid or will that be revoked too?

Yes, it will be valid but it is of no use as you are no longer working with client A.
