DS3035 "Purpose of the form" query

Hi,

 

I was filling the online DS3035 form and wondered what should I choose in the section "Purpose of the form"

 

The DS2019 indicates "Begin new program: Biographical data modified"

 

But, there is no such information in the online form except "New Program"

 

I changed the university means "Program transfer" but the DS2019 indicates

Amend a previous form: Site of activity Modified
