Documents needed for Re-entry on H-4 Visa in USA

Hi, 

 

My wife and I traveled in March to the USA. I am on H1 and she is on H4.

 

Now she went back to india for some family emergency. 

 

What all documents will she need for re-entry back in the US. I know she will need my 1-797 but does that have to be an original one or a copy of it should work ?

A copy of your I797 and a valid H4 visa.
