L1B extension - Different visa expiration and I94 expiration dates

I am on L1B visa which expires in Nov 2014. But my I94 expiration date is Nov 2015.

1. When can I apply for visa extension - 6 months before visa expiration or I94 expiration?

2. If I apply for visa extension after my visa expires and my visa extension gets rejected, will I be allowed to re-enter USA to wrap up my things?

3. If my visa extension gets approved then will the new visa expiration date be 2 years after the previous visa expiration or I94 expiration date?

1. 6 months before your I94 expiry date employer can file for L1B status extension.

2. Visa is an entry document. As long as you stay in US there is no need to extend your visa. When employer files for extension it is for extension of your L1B status in US and not visa.

3. Refer 2. I94 would be based on extension petition validity date.
