Problems rescheduling Appointment @Calgary

Hello All

 

I am facing a strange problem rescheduling appointment @Calgary center, I have been reschuduling the dates for the past year since I registered and today when I am trying to reschedule today it is asking for login information, the login page is like this

Site:

Dont kow what to enter here as I don't know username and password for this log in.

Please let me know if you got any solution to this .I am also getting same login page which is very strange.

I hae re scheduled couple of times ,i never got this login page.
