Pls help..H1 transfer (PP) while H1 ext (Reg) pending

Need quick help from experts.

My friend's H1b extension with company A is in regular processing with pending status and her visa is expiring on October 1st 2013. She is in her first 3 years of H1b.

She got offer from Company B and they filing H1 transfer on Oct 1st with start date of Oct 1st in premium processing. They took her extn receipt for bridging I guess.

1. Her joining date will be Oct 28th. Will there be any issue if she joins company B after H1 transfer approval?

2. Will she be out of status from Oct 1st to Oct 28th if she joins Company B?

It is better to join company B after approval. What would be her situation if her company A withdraws extension and revokes her H1B  and H1B petition with company B is denied?
