Old passport name: Last name: blank First name: Rahul Kumar New Passport name: Last name: Sharma First name: Rahul Kumar In the ds-160 "Have you ever used other names (i.e., maiden, religious, professional, alias, etc.)?" should I fill it Yes or No??and if YES what should I fill in other surname and given name field?Thanks!

MurthyIndia_Lawyer Posted September 27, 2013 Report Posted September 27, 2013 Generally, in similar instances the applicant should mark "YES" to the question "Have you ever used other names (i.e., maiden, religious, professional, alias, etc.)?". on DS 160- Non-immigrant visa application form and provide the other names used in the respective column. One should note that the Department of State has clearly instructed the applicant to provide the entire name in Surname Column and 'FNU' (First Name Unknown) in the Given name column, if the applicant has only one name. Moreover, the system does not allow the applicant to move to the next page if any column is left blank. Apart from this, it is better for the applicant to carry relevant document and/or affidavit to evidence both the names refer to the same person.
