I came to US after visiting india on Sep 15th  But i forgot to  fill the Leave end reporting form hence My pay roll didnt ran For Sep month for 15 days and Oct month.

Is this is any issue for my future visa?

I applied L1 extension in Oct 2012 and it got approved for  2 yrs till 2015. But i have applied H1B this year but it is approved with COS this now.

Is my pay roll which was not ran on Sep and Oct will cause any issues for H1B and future visas . Please advise.

Is my pay roll which was not ran on Sep(15 Days) and Oct is the cause of the COS denial.

Your employer has to approve your 45 days of holidays for you either as loss of pay or pay you.

He already paid my salary in Nov end of month pay roll ( this includes Sep 15 days + Oct 30 days + Nov 30 Days). But i dont have pay stubs for Sep month and Oct month.

Is this cause of my H1 COS denied? Can you please let me know how to fix this issue.

There is always a possibility. Why don't you ask your employer's attorney? He would be the best person to ask.
