when the question says "Do you have any earlier Visa rejections" it means when you attend the visa stamping at consulate and got rejected in your passport.

VISA rejection is different from petition rejection. In your case it's petition rejection not VISA rejection.

During our F1 interviews, lot of people got visa on 2nd or 3rd attempt.

when you attend for F1 Visa and you got refused to get the visa then you need to enter that information while filling the DS 160 now

Basically, in other simple words..what it means is - When you go to the US Consulate for F1 Stamping, be it in India or anywhere else, and if the Visa Officer Reject your Visa, then it is counted as Rejection...and you have to mention this on the DS-160 form.

No, visa rejection means when you attend for 'visa stamp' for any type of visa at any consulate and has been rejected after the interview then that's clear rejection.

if the question says any rejections in last 5 years and if it is yes then yes else no. all questions in ds 160 are straight farward

What ws the decision? Did you get approval? Did the visa officer asked you about your previous rejections?
