Affidavit of support

I am a US citizen applying i-130 and i-485 for my spouse who is on H1B (unstamped) and is currently working in USA. I have 2 questions.

 

- do i need to send i-864 (affidavit of support)

- do i need to send i-693 medical examination even though she is in USA for a couple of years
