AOS questions

I am a US citizen applying petition for my wife who is on H1B (unstamped). I have following questions

 

1) I am applying for AOS (i-485) and it has a question asking for Non Immigrant Visa number, consulate and date of visa issued. There is no visa number as satmping is not done. Is it ok to put  EAC receipt # instead of Non Immigrant Visa number and "N/A" (not applicable) for consulate and date of visa issued question.

 

2) does she need to have police clearance certificate ( she has not  left US for 2 and a half years)
