Montreal consular processing

I am processing my family members from Montreal consulate. Now that the dates are current, I am sending my DS 230 forms to MTL. Have a few questions:

1. Is it must that they need to be sent through loomis (courier company), I have no branch with in 3 hours from where we live.

2. Is Police clearance from Indian consulate is enough or do I need to get it from India as well.

3. I never sent fee for my family members to NVC, now Montreal sent me DS-230, but no one has asked me about the fee? Can they forget that important step?
